Before you start:
Important these documents are required for processing:
Submit copies of the following identification:
1. Birth Certificates for all children
2. Social security cards for all household members
3. Picture ID or birth certificates for each adult
These can be submitted either electronically or by visiting the office. Once these steps are completed, the application review process starts moving.
Applications are available and can be submitted in person at any of our offices in Atchison, Wamego, Linn or Holton, Kansas. If you have any questions on the process contact us
Before you start:
- You will need to have all family members' full name, date of birth, place of birth, and social security number.
- You will need to have addresses for all assisted housing you have ever lived in or received assistance from along with the agency information.
- You will need all addresses where you have lived/stayed in the last 5 years. You will need your dates as well as complete contact information for your landlords.
- You will also need the name, complete mailing address and phone number for five (5) individuals you are NOT
related to who can be a character reference for you.

If your household has more than one (1) adult please note each
additional adult household member must complete the (Name of file) forms and submit the signed documents.
